Register with us as a New Patient

When you attend the Practice to register you will be asked to complete a GP registration form and new patient questionnaire form.

The receptionist may offer you a new patient appointment. Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

Practice area

The practice area comprises the ML1 postcode areas of Motherwell and historically Carfin, New Stevenston, Muirhouse approaching Netherton, and Craigneuk.

If you do not stay within our catchment area other GP practices can be found by accessing the button below.

Your medical records may take several weeks to arrive at the Practice.

If you would rather complete your application to register at home, please click the links below to print both the Registration Form and the practice’s New Patient Questionnaire. These should be completed and brought to the practice reception desk with identification to verify your name and address.

If you are unable to attend the practice in person, forms and identification can be presented at reception by a relative or friend. If this is not an option, please call the practice to discuss further.
